Microsoft 365 Forms allows you to quickly create surveys, quizzes, and polls. Question types include multiple choice, text-based, date, 5-star rating, 10-point scale, and matrix. You can add branching logic so that questions will appear only if they are relevant to the respondent. You can also add sections and can share your forms and quizzes with students using any web browser, even on mobile devices.

Note: Microsoft 365 Forms has been used for phishing scams. Please let your students know that they should never provide personal information, such as passwords, in any form or survey. 

• Create formative assessments using branching. Offer extra or different questions to students who respond incorrectly on previous questions, helping them learn and grow.
• Work collaboratively. Create assessments as a team by sharing a quiz draft with other educators.
• Use built-in analytics. Forms offers analytics that help you evaluate your students' quiz results.
• Export data. You can export survey and quiz results to Excel for additional analysis or grading.
